If you're familiar with Dutch Blitz, you're halfway to knowing Perpetual Commotion. It's a speed-based, no-turn-taking, get-rid-of-your-cards-as-fast-as-you-can kind of game.
Except there's a catch: every round an 8-sided die is rolled, and that die roll tells you how this round is being scored.
